I don't have my Polaris open at the moment, but I seem to recall these were
Switchcraft enclosed jacks. Since they're hand wired and not board mounted,
lots of parts could be substituted. However, if you can find some NL114BX
jacks, they'll substitute for any contact arrangement, because they're
stereo closed circuit insulated bushing, so you just ignore the contacts you
don't need, or wire the sleeve terminal to a chassis ground lug if you need
a grounded sleeve. The Switchcraft web site refers to onlinecomponenets.com,
masterdistributors.com, and mouser.com as possible sources.

As to how they're wired, it shows that in the service manual, on the page
that shows the top level wiring diagram.
