hello,
to be honest, i think your module is faulty.
please check the sockets, maybe they don't make
good contact when a plug is inserted.
when the polarity switches are in middle position
then the signals presented to the corresponding jacks are
ignored, so another (rare) possibility could be that the
switches are not functioning.
