I've been searching for this as well.
The practical advantage of doing a single board with all knobs, jacks and switches on the same surface is massive just in terms of reduced wiring, imo.
The best I've come across is this 1 style -
which is switched (nice) but has the ground connector off the side (ouch).
It may be the same as, or similar to, toby electonics UJ-3506.

I'd be interested to hear of other solutions.
In particular, I'd quite like to know what jack doepfer uses e.g. in A-111-5, and where (or if) they may be sourced in small quantities by mere mortals.
