This happens when the 3P's channel in Ableton is set to 'Auto' while changing patches on the 3P. I've never resolved this problem after many attempts.
It also makes automating the CC's impossible. Only in Ableton –– in Bitwig and Logic it seems okay.
The only thing I can think of is you are sending the synth an aftertouch value and not resetting it to zero when the note is lifted. This will shift everything by the At amount. Try turning off AT->DCO freq AT and see if that helps.
