I did not yet try the PE-OS v4.0, but I had some thing similar with an
earlier version. I simply uploaded the OS (not the bootloader) once
again (with wider pauses between the single messages (I work with an
Apple using 500ms pause in SysExLibrarian).

From which version did you upgrade?

If you want to be on the secure side, you should wait, until Murray will
answer.
